Step 1 - Complete and print out the following forms:

• Student Enrollment Form

• Scheduling Information Sheet

​

• Student Directory Information Form

​

Important:  The above forms can be completed on a computer but then printed.  Downloading or saving the documents will ONLY save the blank form and NOT any information you entered.

Step 2 - Bring the following information to the school:

​

• Completed forms (from Step 1)

• Proof of residence **

• Original certified birth certificate

• Immunization record
